Love Death 2: Realtime Lovers Grim reapers, the carriers of death, exist in our world. One of them, known as 10555 ("Sango"), has lost his scythe. In an attempt to recover it from a man named Kuroda he accidentally killed another person. As a result, Sango was reincarnated as a young boy named Mitsuru Kagami. Mitsuru enjoys a pleasurable life, but all changes when he realizes the grim reaper is a part of him.

Love Death 2: Realtime Lovers is a combination of visual novel-like gameplay with simulation elements. The story unfolds automatically through extensive conversations and text descriptions. From time to time the player is prompted to make a choice that would influence the plot. Animated 3D cel-shaded characters appear on still backgrounds during the visual novel part, but there are no real cutscenes or movement.

During most of the scenes the player can opt to interrupt the narrative and switch to the "movement mode", which allows limited first-person navigation in a 3D environment. This mode is essentially a sex simulation, since the goal is always to have sex with a female character. The player can use stimulating drinks and other items to make the girl more inclined to consent. It is also possible to physically assault and rape the girl; however, in such case the relationship will come to a halt and the story will not reach the best ending.
